When booking online for  permanent makeup, if you are unable to secure a date and time you are looking for please don't hesitate to call or text and I'll be happy to see how I can accommodate you. Please keep in mind I am in the comfort of a Physicians Office, so I do not offer weekend or evening hours. 

 

Please pay a $100 deposit to hold the date and time. Your appointment will not be held without Deposit. {The deposit will be in the form of a Gift Card upon purchase}

 

You must give a minimum one week notice if you need to change your date. If the appropriate amount of time is given your deposit will transfer to a new date. If not, your deposit will be lost and a new one will be needed to reserve a new date and time. If you CANCEL  less than 48 hours OR NO SHOW you will also forfeit your deposit. NO EXCEPTIONS. Please make sure your dates are firm. By reading this and paying your deposit, you are agreeing to the terms of this policy. Thank you!

Once you have booked and paid your deposit, please print and fill out the consent forms located on this page and bring with you to your apt. Please READ ALL of it carefully as it also has your instructions of what not to do prior to your apt.

 

If you say yes to any of the questions or on medications, please notify me via email prior to your apt asap, to ensure we can proceed.
